Update contribution guidelines for PR reviews

Adds guidelines for pull request reviews particularly around requesting
reviews and how to handle review threads:

* Only project members are allowed to request reviews from specific
  users.
* Only reviewers are allowed to resolve review comment threads, only
  project members can unilaterally resolve them if reviewers are
  unresponsive.
* Reviewers should thus take care to check for updates and resolve
  their comments if they have been addressed.
* Pull requests authors should try to avoid force-pushes and rebasing
  their branches to a minimum to make it easier for reviewers to track
  changes during review.

## Pull Request Review Guidelines
In general reviews of pull requests are not limited to project members and can be undertaken by contributors and other GitHub users. Project members commonly coordinate reviews of pull requests at their own disgression in line with project needs and available bandwidth.
To reduce possible friction of the review process, a simple set of rules should be followed:
* **Do not** request reviews from specific community or project members yourself.
* It is permitted to ping other GitHub users to notify them of a PR and kindly ask them if they are willing (and able) to review it, all within reason.
* Reviewers might leave comments or ask for changes on specific lines of changed code. Once the comment has been addressed **do not unilaterally resolve** the comment.
* It is entirely the reviewer's decision whether they would consider the changes to be sufficient to address their concerns. Just having addressed a comment, does not resolve it.
* As a reviewer, make sure to regularly review the comments received on your reviews, check the code changes, and resolve your comments once they have been addressed. **Don't leave pull request authors hanging.**
* Project members will usually ping reviewers about unresolved review comments and resolve such comments themselves to unblock PRs from being merged.
* When addressing review comments through code changes, try to avoid force-pushes and use temporary commits which can be squashed later (see below).
* Unless necessary, avoid rebasing a pull request branch unless it is absolutely necessary. Rebasing will pull in changes from the base branch into the "changes since last push" of the pull request, making it harder for reviewers to check what changes have been pushed.
